Records

Medical records must be maintained for all post-surgical animals.

Operative procedures - To be filled out by the investigator describing:

• The surgical procedure.
• Post-operative orders.
• Persons providing post-operative care.
• Persons and phone numbers where they may be contacted in the case of emergency situations during and after working hours.

Case chart - A medical record of daily observations and treatments. All entries should be signed or initialed.

Identification - A cage card listing pertinent information, including the investigator and animal identification, must be affixed to cages when an animal is placed in a recovery room cage.
